How do I know I've done 5 years of residency?

You have to look at the date of your first residence permit and count from there.

You can ask for a time-count certificate if you want to be officially sure of how long you have been resident in Portugal.

Do I need to live in the country to apply for nationality?

You will have to prove that you have lived in Portugal for the five years on which your application for nationality is based

You will also need to be in Portugal and be able to prove it, during the period between the application being submitted and acceptance by the authorities.

Can I keep my nationality?

It will depend on the rules in your country of origin as Portugal allows dual nationality.
