No. You just need to prove that you have been in Portugal for 5 years with a legal residence card issued by the Portuguese State

If you have Social Security or Tax Authority debts?

In this case, as long as you haven't settled your debts, you can't apply for a permanent permit.

Do I gain nationality with a permanent residence permit?

No. They are separate processes. It is true that after 5 years of proven residence in Portugal over a period of 15 years, you can apply for Portuguese nationality.
